Prairie States offers a variety of exceptional wellness programs and services. These include Optimal Health, FiveStar Health, Condition Management, Case Management and Utilization Management.

If you’re unsure of which programs and services are part of your plan, you can find out by logging in to your My Prairie account and going to My Benefits, by consulting your Human Resources Department or by calling us.
